I think you may have to fix number 9 as they're asking for your ingame stats as well, not your outfit. :x And as everyone has mentioned, you need to check over all the information and correctly answer number 15. So, good luck with that. :P

Other than that, once you fix those, you should be accepted into the clan. ;)
